We are seeking a highly skilled and dedicated professional to fill the position of Indigenous Stepped Care Program (ISCP) Coordinator.

This role will provide Aboriginal and Torres Strait Islanders with or at risk of mental health illness tailored supports through the ISCP program within our organization.

About the Role:

The ISCP Coordinator will support the Stepped Care Mental Health program by arranging services for clients according to care plans in consultation with health professionals. This will involve preparing brokerage request forms and correspondence to stakeholders, including clients and care team members.

Liaising with external stakeholders to promote referral pathways and enhance services is also an essential part of this role. Additionally, the ISCP Coordinator will coordinate activities to meet operational requirements.

Key Responsibilities:
  • Arrange services for clients based on care plans developed in consultation with healthcare professionals.
  • Prepare and send brokerage requests and correspondence to relevant parties, including clients and healthcare teams.
  • Establish relationships with external partners to facilitate referrals and service enhancements.
  • Coordinate activities necessary for operational efficiency.
Requirements:

To be successful in this role, you will need:

  • Certificate III/IV in Primary Health Care or a willingness to obtain it.
  • A current driver's license or ability to obtain one.
  • A First Aid Certificate or willingness to obtain it.
  • A Blue Card (Working with Children Card QLD) or willingness to obtain it.
  • A Federal Police Check or willingness to obtain it.
