Registered Nurse - Care Coordinator
Churches of Christ, Residential Aged Care – Bundaberg
Permanent Part Time – 52.5 Hours per fortnight
About the Role:
Churches of Christ Queensland are currently seeking a Registered Nurse – Care Coordinator for our Bundaberg location. This role will see you working within 'Gracehaven' our 115 bed Residential Aged Care home at Bundaberg supporting our staff and residents by providing advanced clinical support, leadership and coordination of clinical care.
The Care Coordinator is a pivotal role tasked with responsibilities such as
- Coordinating and liaising with external providers
- Overseeing and coordinating the completion of baseline and focus assessments
- Developing and reviewing care plans
- Coordinating the collection of accurate clinical and care data for timely submission and reporting.
- Provide clinincal leadership and support our RN's in developing clinical leadership skills
- Assist the Clinical Manager with follow up and provide oversight in quality clinical care
About You
Are you somebody who is passionate about delivering quality Aged Care services to the local community?
Our ideal candidate is
Essential:
- Passionate about Aged Care and providing safe supportive care to the residents
- Has current AHPRA registration as a Registered Nurse (RN)
- Minimum 3 years clinical experience in the Aged Care Sector
- Current driver licence
- Currently holds, or willing to obtain a current Police Check and Blue Card
- Hold valid Australian Work Rights – Unfortunately we are not currently able to sponsor
Desirable:
- Post-graduate qualifications in Geriatrics, Dementia or other Aged Care specialisation
- Currently hold, or willing to obtain a current First Aid and CPR Certificate
- Currently hold, or willing to obtain a current NDIS check
