Job Summary

We are seeking a dedicated and experienced Registered Nurse to fill the position of Care Coordinator.

Key Responsibilities
  • Coordinate and facilitate effective communication with external stakeholders.
  • Oversee and coordinate the completion of baseline and focus assessments for optimal patient outcomes.
  • Develop, implement, and review comprehensive care plans that cater to individual patient needs.
  • Collect and analyze accurate clinical and care data to inform timely submission and reporting.
  • Provide leadership and support to RNs in developing their clinical skills and expertise.
  • Collaborate with the Clinical Manager to ensure high-quality patient care and adhere to regulatory standards.
Requirements
  • A passion for delivering safe and supportive care to residents.
  • Current AHPRA registration as a Registered Nurse (RN).
  • Minimum 3 years of experience in the Aged Care sector.
  • Current driver's license.
  • Valid Australian Work Rights – unfortunately, we cannot sponsor at this time.
  • Hold a current Police Check and Blue Card or be willing to obtain one.
Desirable Qualifications
  • Post-graduate qualifications in Geriatrics, Dementia, or other relevant Aged Care specialisations.
  • Current First Aid and CPR Certificate or willingness to obtain.
  • NDIS check or willingness to obtain.
