Our organisation is seeking a dedicated Community Liaison to connect residents with locally based services and enhance their wellbeing and independence.

A unique opportunity exists for a skilled professional to join our team as a Community Liaison. Reporting to the Team Leader, you will work closely with community connectors and neighbourhood hubs to develop partnerships and deliver integrated service delivery principles.

In this role, you will provide short-term support to individuals and their families, enabling them to engage with services and supports that meet their needs. You will also foster collaborative relationships with internal departments and external agencies to promote available resources and identify gaps in service provision.

To be successful in this position, you will require:

  • A degree or diploma qualification in a related field, with relevant experience, or lesser qualifications accompanied by substantial experience.
  • Highly developed customer service skills, demonstrating respect and understanding of diversity and difference.
  • Demonstrated skills and knowledge working in community care, enhancing an individual's independence.
