**Clinical Aged Care Needs Specialist Role**

We are seeking a skilled clinical assessor to join our team in Northern and South East Sydney. As a clinical needs specialist, you will be responsible for conducting face-to-face assessments with older Australians in their homes.

Your key responsibilities will include:

- Travelling to complete assessments within designated areas and utilising your clinical expertise to assess the needs of older clients.
- Undertaking comprehensive assessments with a strength-based approach, taking into account Wellness, Reablement, and increasing independence for older individuals.
- Conducting regular client reviews to stay up-to-date with changing needs and circumstances.
- Completing assessments and support plans within set deadlines.
- Facilitating connections between clients and relevant service providers and community support organisations.

To succeed in this role, you will require:

- Registration as a nurse or allied health professional, such as an occupational therapist, social worker, or physiotherapist.
- Current public liability insurance.
- A certificate in aged care Clinical Assessor qualification or willingness to obtain prior to delivering services.
- A good understanding of My Aged Care and the Integrated Assessment Tool (IAT) or ability to learn quickly.
- Detailed knowledge of My Aged Care, related aged care programs, and services with proven ability to apply this knowledge in assessing and supporting complex presentations.
- High level computer literacy, including proficiency in typing and MS Office programs.
- A reliable vehicle and a full current driving licence.
- Current registration with AHPRA or relevant registration body.

A desirable skill set includes:

- Experience in assessment and goal-oriented support planning in aged, community, health, or social services, with a background in coordinating services.
- Strong communication and interpersonal skills to engage and motivate clients, families, staff, and stakeholders with a positive attitude.
- A proactive, self-motivated individual with the capacity to follow due process.
- Excellent teamwork and communication skills.
- Fluency in a second language is an advantage.
- Ability to work on online platforms such as My Aged Care portal.

Job Type: Casual
Pay: $35.00 - $40.00 per hour
Expected Hours: 14 - 16 per week
Work Location: On the road
