As a dedicated professional in aged care, we are seeking an individual to fill the role of Needs Assessor.

About the Position

This is an exceptional opportunity to contribute to delivering high-quality care and services to older individuals.

Key Responsibilities:

  • Conduct thorough assessments of clients referred via My Aged Care Gateway using the Integrated Assessment Tool (IAT).

  • Establish tailored goals for each client, develop strategies and actions, and facilitate referrals to suitable age care and non-aged care services.

  • Complete the IAT and match clients with appropriate service(s).

The successful candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively within a collaborative team environment. They will also have the necessary computer skills to learn and utilize electronic reporting systems.

Required Skills and Qualifications

The ideal candidate will have experience working with older adults and/or hold a relevant certification such as Certificate III in Home Care or Aged Care or Allied Health Assistance.
