Job Description:

We are seeking a highly skilled and experienced Aged Care Clinical Needs Specialist to join our dynamic team at Northern Sydney Local Health District (NSLHD).

About the Role:

This is an exciting opportunity for a dedicated and compassionate professional to work with older persons, assessing their needs and developing personalized care plans.

Key Responsibilities:

  • Conduct comprehensive assessments of older adults to identify their clinical needs and develop appropriate care plans.
  • Work collaboratively with multidisciplinary teams to ensure seamless delivery of aged care services.
  • Develop and maintain strong relationships with clients, families, and healthcare providers to ensure person-centered care.
  • Stay up-to-date with the latest developments in aged care, including policy changes and best practices.

Required Skills and Qualifications:

  • Degree in a relevant health discipline (e.g., nursing, occupational therapy, social work, physiotherapy) with full registration or eligibility for membership of the relevant Australian peak professional body.
  • Proven experience working in aged care settings, with a strong understanding of My Aged Care and related programs and services.
  • Excellent written and verbal communication skills, with the ability to produce high-quality reports and documents.
  • Proficiency in Microsoft Office and electronic patient record systems.
