Job OverviewWe are seeking a dedicated and compassionate Case Manager to provide client-centered support to young people who are experiencing homelessness or at risk of being homeless.Key Responsibilities:
  • Conduct thorough assessments to determine eligibility and support needs
  • Develop, implement and review individualised case plans addressing living skills, welfare needs, interpersonal skills, health, education, training, employment options, social skills, recreational needs and family relationships
  • Provide specialist advice and share knowledge of local networks while maintaining positive internal and external stakeholder relationships
  • Support program activities including completing reports, statistics, feedback summaries and yearly outcomes
Requirements for Success
  • Relevant degree or diploma with at least 2 years' experience working with young people
  • Demonstrated ability to provide specialised therapeutic interventions and manage complex cases
  • Experience with clients dealing with issues including homelessness, poverty, abuse, mental health and substance misuse, and knowledge of support services
  • Highly developed administrative skills along with well-developed written and oral communication
  • Understanding of factors that support recovery and proven ability to positively support others on their recovery journey
