Job Title: Clinical Health Specialist - Community Care

We are seeking a highly skilled and dedicated Registered Nurse to join our clinical team as a Clinical Health Specialist - Community Care. This role will provide health promotion and education services to clients and carers, promoting independence and autonomy.

Estimated Salary:

$85,000 - $110,000 per annum, depending on experience and qualifications

Job Description:Key Responsibilities:
  • Provide health promotion and education services to clients and carers
  • Promote and deliver award-winning Wellness for Independence programs
  • Monitor and promote health outcomes via Telehealth
  • Provide direct care services across various funding programs
Requirements:
  • Degree-qualified Registered Nurse (Division One) with AHPRA registration
  • At least 1 year of community nursing experience
  • Ability to work systematically and accurately
  • Proficient in electronic technology, including smartphones, laptops, email, internet, and client data management systems
