Job Description

We are seeking a highly skilled Support Coordinator to join our team. As a Support Coordinator, you will be responsible for coordinating and managing Disability Services within the NDIS environment.

You will work closely with NDIS participants to provide Level 1 Support Connection and Level 2 Coordination of Support, ensuring seamless access to services and effective coordination of support plans.

Additionally, you will build strong relationships with key stakeholders, including Other NDIS Providers, community venues, community partners, and allied health care teams. You will foster open communication and collaboration to enhance participant outcomes.

The ideal candidate will have a minimum of 12 months experience in a Support Coordination role, relevant knowledge and sound understanding of relevant legislation surrounding NDIS and other community services, and hold valid certifications such as a Working With Children Check, NDIS Worker Screening Check, and First Aid & CPR certificate.

Key Responsibilities
  • Coordinate and manage Disability Services within the NDIS environment
  • Provide Level 1 Support Connection and Level 2 Coordination of Support to NDIS participants
  • Build and maintain strong relationships with key stakeholders
  • Monitor plan outcomes and expenditure to ensure alignment with participants' plan budgets
Required Skills and Qualifications

The following skills and qualifications are required:

  • Minimum of 12 months experience in a Support Coordination role
  • Relevant knowledge and sound understanding of relevant legislation surrounding NDIS and other community services
  • Valid Police Check
  • NDIS Worker Screening Check
  • Working With Children Check
  • First Aid & CPR certificate
  • Min Cert. IV in Disability or similar
  • Holding Valid Driver's License and Own a Fully Insured Vehicle
