Our team is seeking motivated and suitably experienced Mental Health Social Workers to join our Community Therapy Stream. As a Mental Health Social Worker, you will be accountable for providing quality patient care through comprehensive psychosocial assessments, development of intervention plans, and collaborative practice with the Multi-disciplinary team.
Key Responsibilities- You will respond to referrals to Social Work, participate in Quality Improvement activities, complete all data sets, supervise Gr 1 Social Workers and Social Work students, contribute to Social Work initiatives, and engage in Quality Improvement Activities.
- You will report and feedback as per NDIS policy and mandatory reporting timeframes.
To be successful in this role, you will have:
- Significant experience in undertaking comprehensive psychosocial assessments & interventions.
- Significant experience in practicing patient-centered care.
- Demonstrated experience in prioritizing and independently managing complex social situations.
- A demonstrated commitment shown to ongoing professional development.
- Dedication and demonstrated involvement to Quality Improvement.
- Experience in education, support, and supervision to Grade 1 Social Workers and/or students.
- Experience working with the cultural and socioeconomic diversity of our community.
- Extensive knowledge of relevant community services relative to health social work.
- Knowledge of processes, legislation, and community services related to Child Protection, Family Violence, VCAT, Aged Care, NDIS, Elder Abuse, Homelessness, and Mental Health.
- Skilled at safety planning and managing risk, especially for survivors of Family Violence.
- Competent computer skills, particularly in word processing and internet use, and the ability to learn dedicated software.
