Job Summary

We are seeking a dedicated support professional to join our team at Anchored Care.

This role is ideal for someone who is passionate about supporting individuals with disabilities and helping them achieve their goals.

The successful candidate will have the opportunity to make a real difference in people's lives and work in a dynamic and supportive environment.

Key Responsibilities:
  • Provide individualized support to clients, promoting their independence and well-being.
  • Develop and implement personalized plans to meet clients' needs and goals.
  • Collaborate with clients, family members, and other stakeholders to ensure seamless support.
  • Assist clients with daily living tasks, including personal care, domestic assistance, and social support.
  • Transport clients to appointments and engage them in community activities.
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Certificate in Individual Support or Disability and at least 12 months of relevant experience OR 2 years of full-time experience working in mental health.
  • An understanding of the challenges faced by people with a lived experience of a mental health issue.
  • A current Australian Driver's Licence with Reliable Car & Comprehensive Car insurance.
  • NDIS workers check screening Approval which would entail your yellow & blue cards.
  • Clear spoken and professional communication skills both verbal and written to support your clients.
