
Care Specialist Coordinator

This position involves providing specialist support to individuals with mental health conditions, ensuring they receive the necessary assistance and resources to navigate the National Disability Insurance Scheme (NDIS).
As a key member of our team, you will work collaboratively with clients, stakeholders, and service providers to develop and implement personalized recovery plans.
The ideal candidate will possess excellent communication skills, be able to work independently, and have a strong understanding of the NDIS.
- Provide time-limited support to clients on complex matters
- Coordinate various services to support clients in living fulfilling lives within their communities
- Engage with and manage client key stakeholders, ensuring supports are leveraged and not duplicated
- Support and Empowerment: Assist clients in making their own choices about how they want to live their lives
- Time-Limited Support: Provide temporary assistance to clients on complex matters
- Specialist Coordination: Offer specialist support coordination for complex clients
- Network Development: Work with clients, families, and carers to identify a broad network of supports that may assist clients to use their NDIS plan
- Bachelor's degree in a relevant field (Social Work, Psychology, Disability, Occupational Therapy)
- Maintain membership to a relevant professional body (e.g. AHPRA, AASW)
- Current nationally recognized first aid and CPR certificates (or ability to obtain within the first month of employment)
- Positive Notice Working with Children check (or ability to obtain)
- NDIS Worker Screening Clearance (or ability to obtain)
