Our North Queensland office is under new management and requires a team of experienced and dedicated Care Coordinators to support this continued growth in the region.
Role OverviewThe successful candidates will preferably have a strong background in Community Aged/Disability Care. The role itself is responsible for service development, coordination and delivery, including ongoing client assessments and development of care plans that deliver flexible and personalised in-home support. Direct nursing support to clients is also incorporated within the role, as such a current AHPRA-Nurse Registration is required (EN or RN). Successful applicants will thrive on working in a close-knit team and have a passion for delivering the highest quality in client care. This key role will focus on case management, nursing support, customer service, personal care work where required and general administration.

- Minimum of Diploma in Nursing with a current AHPRA registration;
- Driver's License and comprehensively insured dependable private vehicle;
- Current National Police Clearance; and
- Current First Aid & CPR.
- Good understanding of the Government funding environment for aged care (primarily CDC) & NDIS (preferred but not necessary);
- Good understanding of the hospital discharge environment;

- Excellent communication abilities, professionalism, a good reputation & respect for our staff, clients and their families; and
- It is also essential to have good judgment, quick decision-making, excellent communication, staff management and administrative skills.
