As a Housing Stability Specialist, you will provide case management support in community and residential settings. Your goal will be to help individuals achieve housing stability, social inclusion, and self-mastery. This enables them to sustain tenancy and lead fulfilling lives.

This role incorporates person-centred case management practices, focusing on resilience, trauma-informed care, and strengths-based approaches. You will work effectively with your team to deliver a comprehensive service aligned with our vision, mission, and values.

You will report to the Regional Manager and be based in Ashmore, QLD. This fixed-term position operates on a fortnightly roster from Monday to Friday, 8:00 am - 5:30 pm.

Your responsibilities include:

">
  • Delivering services and support that align with the National Homelessness Stream Model of Care.">
  • Establishing effective relationships with clients to encourage self-determination and participation.">
  • Completing thorough assessments, including needs, risks, and vulnerabilities.">
  • Ongoing assessment of individuals' presenting issues, vulnerabilities, and needs from initial intake to exit.">
  • Implementing case plans with active client participation.">

To succeed in this role, you will require:

">
  • Tertiary qualifications in Community Services, Social Work, or a related discipline. A degree-level qualification is preferred.">
  • Relevant experience in social services, including client assessment and case management.">
  • A current Working With Children Check (QLD Blue Card).">
  • A current First Aid Certificate.">
  • A current Driver's Licence.">

The estimated salary for this position is AU$65,000 - AU$85,000 per annum, depending on experience and location.
