Role Overview

We are seeking a dedicated Transition Support Specialist to join our team. In this role, you will work with young people who have been in out-of-home care, providing them with the support and guidance they need to navigate their journey to independence.

Your key responsibilities will include:

  • Developing and maintaining relationships with young people and their networks, including family, friends, and community members.
  • Providing tailored support services, using a strengths-based approach and case coordination that is consistent with best practice principles.
  • Collaborating with stakeholders, including departmental staff, foster families, carers, and other professionals, to ensure young people receive comprehensive support.

Key Requirements

  • A degree or diploma in social welfare or a related field, and a minimum of two years' experience in a similar role.
  • Comprehensive understanding of the challenges faced by young people in care, including those disengaged from education, training, and employment.
  • Ability to work in a culturally safe and trauma-informed manner, with strong knowledge of Aboriginal culture.
  • Proven experience in providing case management, assessment, counselling, and crisis intervention for vulnerable young people.
