Job Summary


The Enriched Living Supervisor LPN is responsible for assessing the individual needs of residents to ensure all personal care is maintained, medications are taken as prescribed, and the physical, mental, social, recreational, and spiritual needs of residents are being provided for by the programs offered in the facility.


Key Responsibilities



  • Supervises and directs activities of Certified Home Health Aides, including activities of daily living, personal supervision, participation in social and leisure activities, and provision of companionship and emotional support to residents.
  • Monitors delivery of resident care to ensure excellent customer service and quality expectations are met.
  • Performs any and all functions of personal care as needed.
  • Provides ongoing communication to families, residents, and medical management team to ensure appropriate notifications regarding medical conditions and updates.
  • Supervises and provides care to residents who are ill, reporting any changes in condition to the immediate supervisor promptly and notifying family, physicians, and others as needed.
  • Ensures resident medications and supplies are available and in stock, ordering and reordering medications per physician orders.
  • Maintains and monitors accurate records for narcotic substances prescribed for residents.
  • Prepares physician consult sheets for medical appointments.
  • Supervises and assists with processes to facilitate prompt and accurate completion of all documentation, such as MARs, medication documentation, 24-hour reports, incident reports, and task sheets.
  • Attends mandatory in-services and professional development programs as required.
  • Promotes staff development and participation in training and in-services.
  • Monitors and manages direct care staff effectiveness, participating in the evaluation, coaching, and disciplinary process for Certified Home Health Aides.
  • Assists in resolution strategies and implementation of plans to address resident or family complaints as they arise.
  • Reports safety concerns and necessary repairs to the maintenance department.
  • Monitors daily staffing to ensure and assign adequate staff coverage for resident care.
  • Ensures the smooth operation and delivery of all services, responding appropriately and/or taking action to resolve issues in the absence of other department supervisors.
  • Monitors the physical plant for safety, security, and effective operation.
  • Understands the emergency preparedness plan and can verbalize their role during an event.

Job Expectations as a Supervisor



  • Coordinates the work of an assigned group of employees, communicating work instructions, distributing, and monitoring work.
  • Acts as the primary contact for routine issues in their area of responsibility, may be a 'working supervisor' who performs the same work as subordinate staff.
  • Provides technical training and assistance on new equipment or procedures, assisting in ensuring employees are provided with appropriate resources.
  • May provide input to employee performance and alert the manager to employee performance issues.
  • Promotes and role models a culture of service excellence and customer service within their functional area.
  • Creates, promotes, and maintains collaborative relationships and partnerships with peers and staff throughout the organization using active listening, unbiased, and non-judgmental language, and open communication style.
  • Uses effective communication and critical thinking skills during conflict resolution, problem-solving, decision-making, coaching, and other complex dialogues.

Qualifications



  • Current New York State license in good standing as a Licensed Practical Nurse or equivalent experience.
  • Minimum 2 years supervisory experience in a related field desired, preferably in a union environment.
  • High school diploma or higher education required.
  • Familiarity with medical transcriptions.
  • Flexible schedule.
  • Outstanding interpersonal skills and the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory, and mental impairments.
  • Strong leadership skills and ability to work collaboratively with a large team.
  • Ability to lead, guide, and motivate others.
  • Able to work efficiently under time constraints.
  • Ability to promote a healthy, collaborative team environment.
  • Proficient in MS Office and able to learn new software rapidly.
