Job Summary

The LPN Supervisor is a key member of the Enriched Living team at Loretto, responsible for ensuring the highest level of care and comfort for our residents. This role requires a strong leader who can motivate and guide a team of Certified Home Health Aides to provide exceptional personal care and support to our residents.

Key Responsibilities

  • Supervise and direct the activities of Certified Home Health Aides, ensuring that residents receive excellent care and support.
  • Monitor the delivery of resident care to ensure that it meets our high standards of quality and customer service.
  • Perform personal care tasks as needed and collaborate with the clinical team on admissions and re-admissions.
  • Develop and implement care plans that meet the unique needs of each resident.
  • Ensure that all necessary documentation is completed accurately and in a timely manner.
  • Provide ongoing communication to families, residents, and medical management teams to ensure that everyone is informed and up-to-date on resident care.
  • Supervise and provide care to residents who are ill, reporting any changes in condition to the immediate supervisor promptly.
  • Ensure that resident medications and supplies are available and in stock, and order and reorder medications as needed.
  • Manage and process changes to physician orders, providing notification to the pharmacy as needed.
  • Maintain accurate records for narcotic substances prescribed for residents.
  • Assist with the scheduling of necessary medical appointments for residents and prepare physician consult sheets as needed.
  • Supervise and assist with processes to facilitate prompt and accurate completion of all documentation, such as MARs, medication documentation, 24-hour reports, incident reports, and task sheets.
  • Manage and facilitate the orientation process for new Certified Home Health Aides and the annual health and reorientation processes for current employees.
  • Attend mandatory in-services and professional development programs as required.
  • Promote staff development and participation in training and in-services.
  • Monitor and manage direct care staff effectiveness, participating in the evaluation, coaching, and disciplinary process for Certified Home Health Aides.
  • Assist in resolving resident or family complaints as they arise.
  • Actively participate in Quality Assurance Performance Improvement (QAPI) audits.
  • Report safety concerns and necessary repairs to the maintenance department.
  • Monitor daily staffing to ensure adequate staff coverage for resident care.
  • Ensure the smooth operation and delivery of all services, responding appropriately to resolve issues in the absence of other department supervisors.
  • Monitor the physical plant for safety, security, and effective operation.
  • Understand the emergency preparedness plan and verbalize their role during an event.

Job Expectations

  • Coordinate the work of an assigned group of employees, communicating work instructions and distributing and monitoring work.
  • Act as the primary contact for routine issues in their area of responsibility, may be a "working supervisor" who performs same work as subordinate staff.
  • May complete and approve schedules in a timely and efficient manner, ensuring the most efficient utilization of assigned personnel to ensure an optimal workflow.
  • Provide technical training and assistance on new equipment or procedures, assisting in ensuring employees are provided with appropriate resources.
  • May provide input to employee performance and alert the manager to employee performance issues.
  • Promote and role model a culture of service excellence and customer service within their functional area.
  • Create, promote, and maintain collaborative relationships and partnerships with peers and staff throughout the organization using active listening, unbiased, and nonjudgmental language, and open communication style.
  • Use effective communication and critical thinking skills during conflict resolution, problem-solving, decision-making, coaching, and other complex dialogues.

Qualifications

  • Current New York State license in good standing as a Licensed Practical Nurse or equivalent experience.
  • Minimum 2 years supervisory experience in a related field, preferably in a union environment.
  • High school diploma or higher education required.
  • Familiarity with medical transcriptions.
  • Flexible schedule.
  • Outstanding interpersonal skills and the ability to communicate effectively with clients, residents, and families from diverse backgrounds who may have physical, sensory, and mental impairments.
  • Strong leadership skills and ability to work collaboratively with a large team.
  • Ability to lead, guide, and motivate others.
  • Able to work efficiently under time constraints.
  • Ability to promote a healthy, collaborative team environment.
  • Proficient in MS Office and able to learn new software rapidly.
