As a Transition Support Educator, you will foster strong relationships with students, families and agencies to provide tailored support for young people transitioning from primary school to secondary boarding school.

Key Responsibilities:
  • Develop strategic partnerships across agencies to support students from remote communities.
  • Provide case management and ensure access to necessary interventions to support eligible students in re-engaging with learning or other pathways.
  • Moderate communication and mediation between students, parents/carers, school and residential staff and relevant agencies.
  • Collaborate with the Secondary Transition Team to support parents/carers of current Year 7-12 boarding school students.
  • Advocate for students' needs and take action to support enrolment, engagement and retention at school and in the residential facility.

Applicants must possess:

- A relevant teaching qualification.
- Current full registration or provisional registration as a teacher in Queensland.
- An exemption card issued by Blue Card Services for Education and Care Services.

- Pre-service teachers should include completed practicum reports and mentor/referee statements.
