Job Summary

We are seeking a dedicated Wellbeing Professional to join our multidisciplinary team. As a key member, you will be instrumental in supporting clients to maintain their independence and autonomy.

Your primary responsibilities will include developing personalized care plans, coordinating with internal and external stakeholders, and managing individualized budgets. You will work closely with healthcare professionals to advocate for clients and participate in case conferencing and clinical appointments.

To be successful in this role, you will require strong communication and networking skills, as well as the ability to build and maintain relationships with clients, families, and healthcare professionals.

Key Requirements
  • Tertiary qualifications in Nursing or Allied Health (with relevant registration)
  • Previous Case Management Experience (community and aged care experience highly desired)
  • Demonstrated ability to develop and maintain strong networks with internal and external stakeholders
  • Current driver's license and comfortable traveling throughout the region each day
  • COVID-19 Vaccination
