Empowering Seniors in Your Community
We are seeking a skilled and compassionate leader to provide essential support services to seniors. As a leader, you will be responsible for planning and coordinating service delivery, promoting awareness and engagement among seniors, building partnerships with local NGOs and community groups, and recruiting and coordinating service volunteers.
- Lead a Dynamic Team: Supervise and guide a team of professionals delivering aged care services through the Commonwealth Home Support Programme (CHSP).
- Service Delivery Planning: Coordinate service delivery to meet contractual requirements and improve client outcomes.
- Promote Senior Services: Develop strategies to increase awareness and engagement among seniors about available support services.
- Community Partnerships: Foster relationships with local health professionals, NGOs, and community groups to enhance service delivery and access.
- Volunteer Coordination: Recruit, train, and coordinate service volunteers to ensure high-quality support services.
- Client Service Agreements: Coordinate processes for client service agreements, onboarding, assessments, referrals, and addressing service gaps.
