We are seeking a highly skilled and compassionate Care Manager to join our team at St Joseph's, Coffs Harbour. As a key member of our clinical leadership team, you will be responsible for ensuring high-quality clinical care is provided to residents at all times, in accordance with the Aged Care Quality Standards and Catholic Healthcare's policies and procedures.
Key Responsibilities- Provide leadership and coaching to the clinical and support teams to ensure the delivery of quality resident outcomes.
- Work in a safe manner, closely following all policies and procedures, including IPC, to ensure compliance with quality standards.
- Collaborate with the multidisciplinary team to develop and implement care plans that meet the unique needs of each resident.
- Conduct regular audits and quality checks to ensure that care is being delivered in accordance with best practice.
- Develop and maintain strong relationships with residents, families, and other stakeholders to ensure that their needs are met and their expectations are exceeded.
- A current AHPRA registration as a Registered Nurse.
- A thorough understanding of the Aged Care Legislation and Aged Care Quality Standards.
- Demonstrated understanding of AN-ACC assessment processes.
- Comprehensive clinical knowledge and understanding of the aging process, particularly age-related health issues.
- Demonstrated knowledge and understanding of workplace health and safety.
- Previous experience working within a Residential Aged Care setting in a similar role.
- Strong coaching/training skills and the ability to influence others.
- Able to work independently and as part of a multi-disciplinary team.
- Competent IT skills, including full Microsoft suite.
