Care Coordinator

About the Role
The Team Leader plays a vital role in ensuring the delivery of high-quality care to our residents. This involves supporting the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care across the home, while ensuring the individual needs, wishes, and goals of each resident are met.
This is an excellent opportunity for experienced care professionals to take on a leadership role and contribute to the provision of exceptional care to our residents.
Key Responsibilities
- Provide social and emotional support to residents, ensuring care provision is holistic and person-centered.
- Support the coordination of care needs, including medication management, nutrition, and hydration, and pressure care management.
- Lead and supervise care on the shift, within designated wings or areas.
- Perform medicine-related tasks in accordance with scope of qualification, state or territory legislation, regulation, and Calvary policies and procedures.
About You
- Relevant qualifications: Diploma of Enrolled Nursing (with APRHA Registration to administer medication), Certificate IV in Ageing Support, or Certificate III in Individual Support with a willingness to undertake training to obtain required qualifications.
- High level of commitment, integrity, honesty, and trustworthiness.
- Effective listener with the ability to communicate at all levels, both orally and in writing.
- Ability to understand and use information technology and programs, and to learn new skills as required.
- Current National police check.
- Working knowledge of WH&S and Infection Control.
- Desire to join a team committed to the care of others in a mission-based organization.
