Care Coordinator

We are seeking a highly skilled and compassionate Care Manager to join our HammondCare at Home team in Coffs Harbour. As a key member of our team, you will play a vital role in delivering high-quality care services to our clients across the Mid North Coast region.
Key Responsibilities- Develop and implement personalized care plans that meet the unique needs and goals of each client.
- Conduct regular assessments and reviews to ensure the ongoing effectiveness of care plans.
- Collaborate with clients, families, and healthcare professionals to ensure seamless care coordination.
- Provide education and support to clients and their families to promote independence and well-being.
- Manage and maintain accurate records and reports to ensure compliance with regulatory requirements.
- Certificate IV in Aged Care or equivalent qualification.
- Minimum 2 years' experience in a care management or related role.
- Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards, and Commonwealth Home Support Programme.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Strong administrative and computer skills, with proficiency in relevant software applications.
