We are seeking a motivated and experienced Care Manager to join our team at Catholic Healthcare. As a Care Manager, you will be responsible for leading our clinical care staff to deliver high-quality, person-centered care to our residents.
Key Responsibilities:- Develop and implement clinical care plans to ensure the delivery of quality care to residents
- Lead and manage a team of clinical care staff to achieve excellent patient outcomes
- Ensure compliance with Aged Care Accreditation Standards and regulatory requirements
- Collaborate with other healthcare professionals to provide comprehensive care to residents
- Authority to practice as a Registered Nurse with AHPRA
- Minimum 2 years of experience as a Clinical Care Manager in an Aged Care setting
- Extensive knowledge of clinical functions and operations in an Aged Care home
- Superior communication and leadership skills
- Ability to work independently and as part of a team
