Role Overview
The McCusker Nurse is a vital position that offers expert advice, support, and referrals to carers of people living with dementia in metropolitan Perth. They act as a key resource, providing guidance and fostering connections with dementia specific agencies to provide support.
This role is being offered on a Permanent Part-time basis working 45.6 hours per fortnight from 8:30am to 4:40pm based at our Corporate Office in Subiaco or the flexibility of working from home. This roster and hours are flexible covering 3 nominated days each week.
Key Responsibilities:Provide person-centred information, education, and/or counselling, including telephone, written and face-to-face communication to individuals and their caregivers.
Offer expert advice, and resources to support the carers of people living with dementia.
Establish and maintain positive and professional relationships with people living with dementia, their carers, and the broader community.
Provide advocacy, and support for people living with dementia and their caregivers.
Evaluate client needs effectively, conduct telephone interviews, and adapt to varying communication abilities.
Ensure a timely and seamless referral process for clients to external and internal service providers.
Implement a regular survey which will collect, and report on outcome measure data from the service provided.
Maintain accurate client record management and ensure reporting is in accordance with policies and procedures.
Present at professional conferences and workshops as requested, to promote and raise awareness of Amana Living's role in dementia services.
The ideal candidate will be an experienced registered nurse with extensive experience in the aged care sector, particularly supporting carers of people with dementia. They will possess strong knowledge of dementia and related services, along with excellent clinical reasoning and decision-making skills. Reporting to the McCusker Manager, the candidate will be part of a team where they will have the skills to work autonomously.
Candidates should hold a Degree in Nursing, Current APHRA Nursing Registration without restrictions, Western Australian C Class Driver's License, and evidence of COVID-19 vaccination.
