About this Role

This is a key position for a Care Coordinator, supporting individuals living with dementia in social settings.

Key Responsibilities:
  • Monitor contractual hours of services and input accurate data/records for staff, clients and carers.
  • Evaluate service delivery to identify gaps and drive continuous improvement.
  • Coordinate referrals to ensure appropriate services, adhering to guidelines, policies and procedures.
  • Apply a person-centred approach to define client goals and capture feedback.
  • Coordinate support workers and ensure effective staff utilisation.
Necessary Qualifications and Skills:
  • Tertiary qualification in aged care or community services or extensive, relevant experience.
  • Previous experience in the health and aged care industry.
  • Ability to plan and organise workload to meet deadlines.
  • Empathy and experience working with people living with dementia.
  • Excellent interpersonal and communication skills, including stakeholder management.

The successful candidate will be able to work effectively in a team environment and provide exceptional customer service.
