About Us

Alzheimer's WA provides care, support, awareness, and information to assist people living with dementia.

We offer advocacy, education, partnerships, training, consultancy, and holistic person-centred care programs based on a philosophy of understanding the lived experiences of those living with dementia.

Care Coordinators coordinate individualised supports and services for our clients from onboarding through to review to enable the client's needs and goals are met throughout their dementia journey.

Key Responsibilities:

  • Monitoring contractual hours of services within allocated budget;
  • Inputting and maintaining accurate data/records regarding staff, clients, and carers;
  • Ensuring ongoing evaluation of service delivery to identify gaps in service provision and ensure continuous improvement;
  • Assisting in formulating and implementing team procedures;
  • Coordinating referrals to implement appropriate services for people with dementia adhering to guidelines, policies, and procedures;
  • Using re-enablement and wellbeing approach to work collaboratively with clients to define their goals and encourage feedback on how they would like to shape their service;
  • Coordinating support workers to ensure effective staff utilisation in appropriate services to achieve best outcomes for our clients;
  • Providing dementia-specific information to clients and carers as requested;
  • Coordinating services including social support, in-home respite, groups, and day centre services;
  • Providing appropriate feedback to House Manager on any service issues;
  • Liaising with external service providers and agencies to facilitate information sharing and promotion of Alzheimer's WA services.

Essential Criteria:

  • Tertiary qualification in aged care or community services or extensive relevant experience;
  • Previous experience in health and aged care industry;
  • Demonstrated ability to plan and organise workload to meet tight deadlines;
  • Demonstrated empathy, understanding, and experience of working with people living with dementia;
  • Excellent interpersonal and communication skills including ability to manage stakeholder relationships;
  • Valid driver's licence;
  • NDIS Check (or willing to obtain);
  • Up-to-date COVID-19 vaccinations (3 doses).
