Job Title: Registered Nurse
We are seeking a dedicated and experienced registered nurse to provide expert advice, support, and referrals to carers of people living with dementia in metropolitan Perth.
About the Role:- Provide dementia-specific information, education, and/or counselling through telephone, written, and face-to-face communication.
- Establish and maintain positive professional relationships with individuals living with dementia, their carers, and the broader community.
- Offer advocacy and support for individuals living with dementia and their caregivers.
- Evaluate and refer clients seamlessly to external and internal service providers in a timely manner.
- Manage client records effectively and ensure reporting aligns with Amana Living's policies and procedures.
- Implement regular surveys to collect and report outcome measure data from provided services.
- Experience working in aged care, particularly supporting carers of people with dementia.
- Demonstrate strong knowledge of dementia and related services.
- Display excellent clinical reasoning and decision-making skills.
- Able to work autonomously, conduct telephone interviews, assess client needs effectively, and adapt to varying communication abilities.
