Clinical Medication Specialist

We are seeking an experienced Pharmacist to join our dynamic team in a fast-paced clinical environment.
About the Role:- Conduct thorough medication chart reviews and reconciliation
- Manage and dispense medications, as well as supply pharmacy products
- Coordinate patient medication needs for inpatient stays, day infusions, and discharges
- Develop strong relationships with key stakeholders to deliver exceptional customer service
- Maintain accurate and up-to-date patient records
- Collaborate with clinicians, nurses, and technicians to ensure effective patient care
- Participate in regular quality improvement initiatives to enhance patient outcomes
