We are seeking a compassionate Client Services Manager to join our team in Darwin, playing a vital role in developing care plans aligned with client needs and aspirations.
You will use your skills and knowledge to conduct assessments and develop person-centred care plans focused on fostering client independence, wellness and reablement.
You will also assist and enable clients and their families to understand how they can effectively utilise their Home Care Package funding.
About this role:
- Develop and implement care and support plans tailored to client needs and aspirations
- Conduct comprehensive assessments and provide recommendations for care services
- Build strong relationships with clients, their families and other stakeholders
- Collaborate with multidisciplinary teams to ensure cohesive care and support
About you:
- A empathetic approach to working with older people
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
- Diploma level qualifications in health/aged care/social welfare (desired)
