Case Manager Role Overview

This role is a permanent position that involves managing a caseload under CHSP/HCP approved funding. The primary objective is to maintain budgets, onboard new clients, and coordinate care to support client independence.


Key Responsibilities
  • Manage a caseload of clients under CHSP/HCP approved funding
  • Maintain budgets and manage care requirements to support client independence
  • Onboard growth of new clients and coordinate with stakeholders
  • Time management
  • Understanding the Aged Care Standards
  • Liaise with stakeholders and internal networks

Required Skills and Qualifications
  • Minimum 1 year experience in case management
  • Relevant tertiary qualification in the community sector
  • Experience in aged/community/disability sector, particularly with clients with complex needs
  • Full driver's license and own reliable registered and insured vehicle
  • Smart phone
  • Knowledge of My Aged Care portal/assessment process
  • Ability to work in a team environment
  • NDIS clearance (or willingness to obtain one)
  • Police clearance (within the last 6 months or willingness to obtain one)
