
Healthcare Service Coordinator

We are seeking a dedicated and skilled Clinical Coordinator to join our team. As a key member of our healthcare delivery team, you will play a vital role in coordinating the delivery of service to clients with complex care needs.
Key Responsibilities:
- Developing and Implementing Client Care Plans: You will be responsible for developing and implementing client care plans that meet the unique needs of each client. This will involve assessing client needs, identifying potential risks and barriers, and developing strategies to mitigate these risks.
- Providing Clinical Guidance: You will provide clinical guidance to clients and support staff to ensure that they have the necessary skills and knowledge to deliver high-quality care.
- Coordinating Service Delivery: You will work closely with our multidisciplinary team to coordinate the delivery of service to clients, ensuring that all aspects of care are met.
- Facilitating Inductions and Training: You will facilitate inductions and training for new staff members, ensuring that they have the necessary skills and knowledge to perform their roles effectively.
- Managing Stock Orders: You will manage stock orders for clients, ensuring that all necessary equipment and supplies are available when needed.
About You
To be successful in this role, you will need to have a strong background in nursing, excellent communication and interpersonal skills, and the ability to work autonomously and as part of a team.
Requirements:
- Current AHPRA Registration: You must hold current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.
- Passion for Health Education: You will have a passion for health education and a commitment to delivering high-quality care to clients.
- Excellent Communication Skills: You will possess excellent communication and interpersonal skills, enabling you to work effectively with clients, colleagues, and other stakeholders.
