
Senior Care Coordinator

As a Senior Care Coordinator with Regis Aged Care, you will play a vital role in developing and implementing personalized care plans that cater to the unique needs and aspirations of our clients.
">- Through thorough assessments and person-centered planning, you will work closely with our clinical experts and allied health professionals to develop tailored care plans that foster independence, wellness, and reablement for our clients.
- You will assist our clients and their families in understanding how they can utilize their Home Care Package funding to achieve positive outcomes that optimize their health, wellbeing, and safety.
- With your empathetic approach and exceptional communication skills, you will build strong relationships with our clients, their families, and our internal stakeholders, ensuring seamless care coordination and support.
- As a key member of our home care team, you will contribute to creating a culture of respect, kindness, and empathy, where our clients feel valued, supported, and empowered to live life on their terms.
To succeed in this role, you will require:
- A Diploma level qualification in Health/Aged Care/Disability/Social Welfare or related field.
- Proven experience working in aged care or a related field, with a focus on client-centered care and service delivery.
- Exceptional interpersonal and communication skills, with the ability to build trust and rapport with clients, families, and internal stakeholders.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage competing demands.
- A current driver's license and willingness to travel to visit clients in their homes (if required).
