Healthcare Support Specialist

We are seeking a compassionate and organised individual to support customers through their care journey.
This is a full-time role where you will guide customers through the process of developing holistic health plans, coordinating services, and ensuring safe, person-centred care.
Key responsibilities include:
- Building trusted relationships with customers and their families
- Conducting onboarding assessments and developing comprehensive care plans
- Coordinating multidisciplinary services and collaborating with clinical teams and external providers
- Maintaining accurate records and participating in quality improvement initiatives
Requirements:
- Experience in community or aged care
- Strong customer focus and excellent communication skills
- Ability to work effectively in a team-based environment
- Relevant qualifications desirable
