We are seeking a passionate and caring individual to provide intensive support and assistance to vulnerable older Australians to understand and access aged care services as well as advocate for those unable to access aged care services without support.
Key Responsibilities- Promote Catholic Healthcare's value proposition and service offering in the community to ensure clients and community groups have an understanding of aged care services.
- Be a professional brand ambassador to the community, working with client advocates, referral providers, and understanding program requirements.
- Engage with specialized services and attend and present at community events, forums, and expos.
- Ensure a strong understanding of client needs, requirements, and potential by monitoring the client's circumstances and maintaining relevant client records to support appropriate referral and maintain care once assigned.
- Attend regular meetings, including monthly community practice meetings and other face-to-face, phone-based, multidisciplinary meetings (travel may be required).
- Provide direct and indirect support to clients, including engaging and building rapport and support to interact with My Aged Care (register, eligibility, book in assessment).
- Work with vulnerable program participants, including those who are isolated, facing communication barriers, including limited literacy skills, and/or resistant to engage with services and providers to access care.
- Genuine passion for providing high-quality care to the elderly and experience with vulnerable communities (or similar).
- Driver's license and a reliable insured vehicle.
- Qualification in Community Services, Social Work, or equivalent experience.
- Strong communication skills and a heart for the community.
- Excellent digital literacy and learning agility with new systems.
- Willingness to apply for a Police Check.
- Competitive hourly pay rate + paid travel time.
