About the Role:

We're seeking an experienced Accredited Practising Dietitian to provide specialist telephonic nutrition care and care coordination to our members through comprehensive person-centred care planning or utilising an existing suite of contracted service providers.

Job Description:

  • Salary: Estimated $80-$110 per hour based on industry standards and location (Surry Hills Health Centre)
  • Provide quality, safe, appropriate information to members with health/care needs
  • Identify and engage Members who may benefit from chronic disease management services through outbound contact processes
  • Conduct telephonic healthcare assessments with Members
  • Person-centred care planning, according to member goals and immediate health needs; referring people with care needs to other service providers as required; regular review to assess outcomes and modify care plans as appropriate

Required Skills and Qualifications:

  • Accredited Practising dietitian – current unrestricted registration with Dietitian's Australia
  • Computer Literacy – competency in MS Office suite
  • 5+ years (full-time equivalent) experience as an Accredited Dietitian
  • Experience in Health Coaching
