
Clinical Care Professional

We are seeking a highly skilled and dedicated Clinical Care Coordinator to join our multidisciplinary team. As a key member of our team, you will play a vital role in delivering high-quality evidence-based clinical care to adults aged 18 years and over who experience mental health concerns and their families and carers.
The successful candidate will be responsible for conducting intake and assessments, including risk assessments, care treatment planning, care coordination with internal & external services, and providing brief psychological intervention.
They will also work collaboratively with other healthcare professionals to ensure seamless delivery of care and achieve better health outcomes for our clients.
Key Responsibilities:- Conduct intake and assessments, including risk assessments, care treatment planning, and care coordination
- Deliver evidence-based clinical care as a member of the multidisciplinary team
- Work collaboratively with other healthcare professionals to ensure seamless delivery of care
- Provide brief psychological intervention as needed
To be successful in this role, you will need to have:
- A relevant qualification in Occupational Therapy, Mental Health Nursing, Psychology, or Social Work
- Full registration with AHPRA as an Occupational Therapist or Mental Health Nurse, or General/Provisional registration as a Psychologist/Clinical Psychologist, or AASW registration/eligibility as a Social Worker
- Excellent communication and interpersonal skills, with the ability to engage with people, advocate on their behalf, and establish and maintain effective relationships with staff, clients, families, and friends
- Highly developed knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice, particularly in the areas of mental health and alcohol and drug use
- Experience working in a clinical setting, preferably in mental health
- Ability to use electronic client/customer management systems and Microsoft Office applications
