Job Title: Clinical Care Coordinator

Deliver high-quality evidence-based clinical care as a member of a multidisciplinary team to achieve better health outcomes for adults aged 18 years and over who experience mental health concerns and their families and carers.

  • Conduct intake and assessments, including risk assessments, care treatment planning, care coordination with internal and external services, and provide brief psychological intervention.
Required Skills and Qualifications

The ideal candidate will possess:

  • One of the following: Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist OR AASW registration/eligibility as a Social Worker.
  • Flexibility to work afternoons and evenings, weekends, and public holidays.
  • Knowledge and understanding of theory underpinning evidence-based interventions and clinical practice as well as demonstrated excellence in practical application of such in areas of mental health and alcohol and drug use.
  • Highly developed communication (including written) and interpersonal skills.
  • The ability to engage with people, advocate on their behalf, and establish and maintain effective relationships with staff, clients, families, friends, and stakeholders.
  • Experience in using electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
  • Eligibility to apply/currently hold a WWCC.
  • Willingness to apply for NDIS worker screening clearance.
