Job Title: Clinical Care Coordinator

We are seeking a highly skilled and experienced clinical care professional to join our multidisciplinary team.

Role Overview:
  • Deliver high-quality, evidence-based clinical care to adults aged 18 years and over who experience mental health concerns and their families and carers.
Key Responsibilities:
  • Conduct intake and assessments, including risk assessments, care treatment planning, coordination with internal & external services, and provide brief psychological intervention.
Requirements:
  • Must have one of the following qualifications: Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist OR AASW registration/eligibility as a Social Worker. Must also possess flexibility to work afternoons & evenings, weekends and public holidays.
  • Knowledge & understanding of theory underpinning evidence-based interventions & clinical practice, as well as demonstrated excellence in practical application, in areas of mental health and alcohol and drug use.
  • Highly developed communication (including written) and interpersonal skills.
  • The ability to engage with people, advocate on their behalf and establish/maintain effective relationships with staff, clients, families & friends and stakeholders.
  • Experience in electronic client/customer management systems, desktop computer usage, and Microsoft Office applications.
  • Eligibility to apply/currently hold a WWCC.
  • Holding/be willing to apply for NDIS worker screening clearance.
