About the Role

We are seeking a dedicated and compassionate Home Care Assistant to join our team in the Merredin region. As a Personal Care Worker, you will provide holistic, client-focused care in line with each client's tailored care plan.

Key Responsibilities:
  • Provide personal care and assist with daily activities, following the Client Care Plan.
  • Work closely with Registered Nurses to deliver client-specific care, including personal hygiene, toileting, nutrition, mobility, and more.
  • Identify client care needs and work collaboratively with the client's care team to ensure high-quality, holistic care.
  • Communicate effectively with clients, their families, and team members.
  • Support clients with light housekeeping duties to promote independence, including laundry, cleaning, and meal preparation.

Required Skills and Qualifications:
  • Aged Care Certificate or equivalent qualification.
  • Previous experience in a similar role, preferably in a home care setting.
