We are seeking a Support Coordinator to fill this pivotal role in empowering individuals with disabilities to achieve their goals.
Your Key Responsibilities:
• Collaborate with customers to develop skills and confidence needed to manage their NDIS plan
• Serve as the primary liaison point for all related services selected by the individual
• Foster a customer-centric approach to enable others to build confidence in their capabilities
• Act as a key support system, guiding participants towards taking ownership of the Support Coordination function over time
