Job Description

Nurse Care Specialist

As a Nurse Care Specialist with our organization, your primary responsibility will be to deliver high-quality patient care services. This will involve conducting thorough assessments, developing individualized care plans, and implementing evidence-based interventions to promote optimal health outcomes.

You will also have the opportunity to work collaboratively with multidisciplinary teams to coordinate care, address complex issues, and facilitate communication among healthcare providers.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  • Develop and implement individualized care plans that align with patients' goals and values.
  • Provide direct patient care, including medication management, wound care, and other treatments as prescribed.
  • Maintain accurate and detailed records of patient interactions, including progress notes, treatment plans, and discharge summaries.
  • Collaborate with healthcare providers, families, and caregivers to ensure seamless transitions of care and optimize patient outcomes.
Required Skills and Qualifications:

Education and Certification:

  • Degree in Nursing (BSN or MSN)
  • Current licensure as a Registered Nurse (RN) in good standing
  • Basic Life Support (BLS) certification

Experience and Skills:

  • Minimum 1 year of experience as a Registered Nurse in an acute or long-term care setting
  • Proven ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop creative solutions
