Oncology Care Navigator

As an expert in the specialty field, you will provide advanced level nursing services to support patient management. You will collaborate with Local Health District nurses/midwives and medical officers to ensure evidence-based practice is utilised.
This role involves assessing, planning and reviewing patients in collaboration with primary service providers. The successful candidate will lead clinical organisation, coordination, leadership and consultancy services within the oncology service and target populations within the Northern NSW Local Health District.
- Providing expert care coordination of patients receiving concurrent Chemoradiation therapy.
The successful candidate will work closely with local health professionals to ensure high-quality patient care and outcomes. This role requires a strong understanding of oncology care principles and practices.
Key responsibilities include:
- Coordinating patient care and services across multiple healthcare settings.
- Developing and implementing individualised care plans to meet the unique needs of each patient.
- Collaborating with multidisciplinary teams to ensure comprehensive patient care.
- Maintaining accurate records and reports on patient care and outcomes.
