Job Title: Advanced Cancer Care Coordinator

This is a unique opportunity to provide expert care coordination services for patients receiving concurrent Chemoradiation therapy.

About the Role
  • Provide assessment, care planning and review of patients in collaboration with primary service providers.
  • Facilitate linkages and relationships with nurses/midwives and medical officers to support patient management.

As an advanced level nursing professional, you will lead expert clinical organisation, coordination, leadership and consultancy services within the oncology service and target populations within Northern NSW Local Health District (NNSWLHD).
