**Job Title:** Home Care Services Coordinator

**Job Description:

We are seeking a highly skilled and dedicated individual to join our team as a Home Care Services Coordinator. This is an exciting opportunity for someone who is passionate about delivering high-quality care services to clients in their own homes.

This role involves managing the delivery of home care services, including assessing client needs, developing care plans, and coordinating with service providers. You will work closely with clients, their families, and healthcare professionals to ensure that our clients receive the best possible care.

To be successful in this role, you will need excellent communication and interpersonal skills, as well as the ability to work independently and as part of a team. You should have a strong understanding of the principles of person-centered care and be able to demonstrate empathy and compassion when working with clients.

Key Responsibilities:

  • Assessing client needs and developing individualized care plans
  • Coordinating with service providers to deliver high-quality care services
  • Monitoring and evaluating the effectiveness of care plans and making adjustments as needed
  • Providing education and support to clients and their families on care-related topics

Requirements:

To be considered for this role, you will need:

  • A certificate III in Individual Support (or equivalent) - Aged Care, Disability or Home and Community Care
  • At least 3-5 years' experience working in a community-based care setting
  • Excellent communication and interpersonal skills
  • The ability to work independently and as part of a team
