**The Role**:
This role requires a person-centred approach to develop customer service, manage individual budgets, support plans and goals while promoting choice, control and independence. Allowing our clients to continue living their best lives at home the role encompasses ongoing coordination / support for Home Care Package Clients as well as the assessment and review of client's choices and needs.
This is a permanent fulltime position.
**About You**:
To be successful in the role you will have a passion for customer service and understand the value to older people to remain in their own homes.
As someone who possesses excellent communication skills both written and verbal. With a natural ability to adapt to the client's style.
As the right person for this role you will have a high degree of empathy and an ability to prioritize your work.
**Qualifications/Experience**:
- A tertiary qualification in community or aged services, Registered Nursing, Enrolled Nursing, Social Work and/or equivalent industry experience
- Working knowledge of the My Aged Care processes
- Sound understanding of the Aged Care Quality Standards
- Excellent decision-making and problem-solving skills to manage the provision of quality services, whilst balancing the choices of clients, staff and the organisation
- Strong interpersonal skills and a continuous improvement approach
- COVID and Influenza Immunisation Certificate
- Satisfactory National Police Clearance
- Current SA motor vehicle driver's license
- Computer literacy is essential - Knowledge of CIM is highly desirable
