Job Title: Home Care Coordinator

We are seeking a dedicated and compassionate individual to join our team as a Home Care Coordinator.

The successful candidate will be responsible for providing high-quality home care services to our customers, ensuring they receive the best possible support to maintain their independence and live safely in their own homes.

  • Provide personalized care plans tailored to each customer's needs and preferences.
  • Coordinate and manage home care packages, ensuring timely delivery of services and effective budget management.
  • Collaborate with customers, families, and healthcare professionals to ensure seamless care coordination.
  • Develop strong relationships with customers, understanding their unique needs and circumstances.
Required Skills and Qualifications

To be successful in this role, you will possess:

  • A qualification in nursing or an allied health discipline (preferably).
  • Demonstrated experience in a clinical, community health, or case coordination role.
  • Excellent communication and interpersonal skills, with the ability to build rapport with customers and stakeholders.
  • Proven problem-solving skills, with the ability to respond flexibly and positively.
  • A commitment to delivering high-quality customer service and achieving excellent outcomes.
