
Home Care Coordinator

We are seeking a dedicated and compassionate individual to join our team as a Home Care Coordinator.
The successful candidate will be responsible for providing high-quality home care services to our customers, ensuring they receive the best possible support to maintain their independence and live safely in their own homes.
- Provide personalized care plans tailored to each customer's needs and preferences.
- Coordinate and manage home care packages, ensuring timely delivery of services and effective budget management.
- Collaborate with customers, families, and healthcare professionals to ensure seamless care coordination.
- Develop strong relationships with customers, understanding their unique needs and circumstances.
To be successful in this role, you will possess:
- A qualification in nursing or an allied health discipline (preferably).
- Demonstrated experience in a clinical, community health, or case coordination role.
- Excellent communication and interpersonal skills, with the ability to build rapport with customers and stakeholders.
- Proven problem-solving skills, with the ability to respond flexibly and positively.
- A commitment to delivering high-quality customer service and achieving excellent outcomes.
