Job Title: Home Care Coordinator
Job Description:
We are seeking a skilled professional to join our team as a Home Care Coordinator. As a Home Care Coordinator, you will be responsible for managing a portfolio of community-based elderly customers with home-care packages.
The key responsibilities include sales focus, building relationships with customers and their families, developing tailored care plans, managing customer budgets, and coordinating care services to maintain independence.
You will be inspired to make a positive difference in the lives of our customers and achieving ultimate customer satisfaction.
Work Schedule:
Your week will be spent mostly visiting customers in their homes, while liaising closely with internal teams to ensure the smooth delivery of care services.
About You:
Previous case management experience is essential, particularly in Home-Care Packages. Advanced knowledge of the Community/Aged Care system and funding model is also required.
A valid driver's license and reliable vehicle are necessary. Strong coordination and organisation skills, exceptional customer service, negotiation and influencing skills, and excellent ability to connect with multiple stakeholders are necessary.
