As a vital healthcare professional, you will play a critical role in our Extra-Mural Program. Your responsibilities will include coordinating patient care holistically, assessing, planning, implementing, and evaluating patient needs.

Main Responsibilities:
  • Coordinate patient care by incorporating nursing values of knowledge, theory, and practice
  • Assess, plan, implement, and evaluate patient needs through collaboration with interdisciplinary teams
  • Work efficiently as a team member, demonstrating leadership and coaching skills, strong interpersonal skills, and good judgment and decision-making skills

You must possess a Diploma in Nursing or a Bachelor Degree in Nursing, be registered (active) with the relevant nursing association, and have at least two years of specialized nursing experience in family practice, surgical, palliative care, or oncology.

Additional qualifications include knowledge of the target population, a valid driver's license, a clean criminal record check, and current CPR certification.
